A 29 year old lady presents with a recurrent bony swelling in her cheek bone for last 14 years. She had undergone curettage of bony tumor elsewhere and presented with a recurrent swelling. She underwent total maxillectomy with reconstruction of hard palate with micro vascular anterolateral thigh flap. A skin island with underlying muscle cuff from the thigh was taken and its vessels were anastomosed to the vessels in the neck. This allowed for a complete recontruction of the palate with maintenance of good contour of cheek and face.
